Pressure care needs can change over time.
Someone may start out doing well on a stable mattress, then later spend more time in bed, lose mobility, or develop more fragile skin. A pressure injury may also progress from Stage 1–2 to Stage 3–4. When that happens, the support surface may need to change too.
That is where hybrid mattresses can work well. They allow care to start simply, then step up later if needed.

Why start with a static hybrid?
A static hybrid mattress can be a good starting point when the person needs more than basic foam, but does not yet need full alternating therapy.
That matters in home care, palliative care, and progressive conditions, where comfort, quiet, and familiarity often matter just as much as pressure redistribution.
Starting with E400 can make sense because it offers:
- a calm, stable surface
- a more familiar bed feel
- better pressure care than standard foam
- less bedside equipment for families and carers
In short, it helps protect skin without making the setup feel too clinical.

Why not jump straight to full alternating air?
Sometimes a full alternating mattress is the right choice. But not always.
Starting too early with a more active surface can bring extra noise, movement, and complexity. For some people, that affects sleep, comfort, and transfers.
That is why “more advanced” is not always the best first step.
If the person still tolerates a reactive surface well, a static hybrid may be the better place to start.
At a glance: why this pathway works
Start with E400 when:
- risk is present, but not yet extreme
- comfort and routine matter
- the setting is home, RACF, or palliative care
- the person may need more support later, but not yet
Step up to E400 + EP10AU when:
A few signs may suggest E400 alone is no longer enough:
- the person is spending much more time in bed
- repositioning has become harder
- skin condition is worsening
- a pressure injury has progressed from Stage 1–2 to Stage 3–4
- overall condition has declined
That does not mean the first setup was wrong. It means the support needs have changed.
Why that helps:
- keeps the same mattress
- reduces disruption for the person
- avoids unnecessary equipment changes
- gives carers and family more continuity and confidence
This is especially relevant in progressive conditions, where needs often change gradually. The person may not need a major mattress change right away, but they may need one later.
